Description: I'm looking for the chosen provider to create a simple Lead Capture page based up on the design I already have created. (see attached)

I'm just looking for the design to be created into a simple lead capture page website with an opt-in form. I will include video instructions on how I want it laid out.

For the right person with basic website design skills it should not take long at all.

The ideal person for this job will have excellent reviews on GAF with a proven record of getting projects done in a timely manner.

I'm looking to get this done ASAP so you must be ready to start this job immediately.
